Step 2: How to complete online forms:

Step 3: How to print the form
Once you have completed all items online, you are ready to print out a final copy. You can not save the completed version. Be sure to print out a copy while still online.

Step 4: Mail or file your form in person
Obtain the required signatures in the presence of a notary public and then file your form together with a $5.00 check payable to the McHenry County Clerk's Office.
